Eligible for the NDIS
When determining your eligibility, the NDIS considers:
- your age
- your disability or significant impairments
- whether significant impairment is permanent or likely to be permanent
- whether you would benefit from early
To be eligible you must
- Be under 65 years of age
- Be a Australian Citizen or Permanent Resident of Australia
- Have an impairment, condition or diagnosed disability that is likely to be permanent (lifelong) and that stops you from doing everyday things by yourself.
- If treatment is no longer suitable in assisting you
Funded supports in a NDIS plan will be provided to you so you are able to live an independent life to best of your abilities.

What is the “Access Request Form”?
To determine if you are eligible for the NDIS you will need to commence the process by calling the National Disability Insurance Agency (NDIA) on 1800 800 110 and ask for an Access Request Form.
They may ask you a series of questions over the phone before sending you further documentation which will need to be completed.
